Important Changes Announced for Sports Physicals and Clearance to Play on Sports Teams

Before a student can try-out or participate in an interscholastic sport, each season (fall, winter, spring), s/he must be medically cleared through the Health Office.

For medical clearance, a student must submit the following:
A current physical exam performed within the last 12 months
A Sports Clearance Packet, completed as indicated 
Health History, completed by the parent/guardian
Emergency Clearance Card, completed by the parent/guardian
Athlete Standards and Consequences Form signed by both parent/guardian and athlete

In past years, the District physician would be available to do required sports physicals in school.  The number of sports physicals provided by the District's physician has grown significantly in recent years. The 2009-10 budget was exceeded by winter and there are no funds left to perform any more physicals this year.  Also, this budget line was greatly reduced for 2010-2011.
For this reason, the school will not be scheduling any physicals.  Parents are urged to make appointments with their family physician as soon as possible to obtain the required physical exam.  The high school health office can provide names of local doctors if you need them.
For all high school fall sports, the Sports Clearance Forms must be returned to the health office between August 9 and August 19. If they are not in, your son will not be allowed to practice!!

For all middle school (modified) fall sports, the forms must be submitted between August 9 and August 31. Forms signed before August 9 will not be valid, per NY State guidelines.
